Overview

76% of India's clinics run on paper and shouting 1.5 million neighbourhood clinics issue handwritten tokens. Patients wait 2–3 hours with zero visibility. Doctors have no dashboard. Receptionists manage everything from memory — and the paper system persists because it is fast. Any digital replacement must be faster, not just shinier. Process Start with the constraint: faster than paper The real insight was that the paper system is not broken because it is paper — it is broken because it provides zero information. The solution had to match paper's speed and beat it on transparency. 1 Defined the 10-second constraint Timed a real receptionist writing a paper token — 3 seconds. Set the target: digital registration must be ≤5 seconds. Designed the form around keyboard-only flow with Enter to submit and auto-focus after each entry. 2 Chose shared state over polling Instead of building a server with polling, I put both screens in one JS state object. Call next → state mutates → both DOMs update in one frame. Zero latency, demonstrable in the prototype without a backend. 3 Designed the wait time formula Hardcoded averages are wrong f Results Registration speed ~4 sec name → Enter → done. No mouse needed. Reflection Build the WebSocket server from the start. The shared-state approach proves the concept perfectly but means the demo can't run on two real devices simultaneously. A 30-line Node.js + Socket.io server would have made the live sync story bulletproof across phones and TVs — not just browser tabs. Add SMS notification at 2 tokens away. The biggest anxiety for a waiting patient is stepping outside.
